This form needs to be signed by a student or faculty member or graduate IF they have NOT been paid to produce the artwork. For instance, if a student, faculty member, or graduate redesigned the Starbucks logo as a freelance job, and were paid to do so, the Student Permission form would not be appropriate. They would need to sign the Artwork Permission form. |

This form needs to be signed by the owner of the artwork (who also paid to have the work done), even the work was done by a student, faculty member or graduate. For example, there is a The Art Institute of Pittsburgh grad who took photographs that were published by The Pittsburgh Post-Gazette. We needed to secure permission from the newspaper, not the grad. |
| The Photo/Model Release form |
This form can be used for all interviews and b-roll shots. But if any student artwork is seen in these shots, the Student Permission form or the Artwork Permission form must be signed, depending on the situation. |
