Student forms
The Student Permission form. This form needs to be signed by a student or faculty member or graduate IF they have NOT been paid to produce the artwork. For instance, if a student, faculty member, or graduate redesigned the Starbucks logo as a freelance job, and were paid to do so, the Student Permission form would not be appropriate. They would need to sign the Artwork Permission form.
The Artwork Permission form. This form needs to be signed by the owner of the artwork (who also paid to have the work done), even the work was done by a student, faculty member or graduate. For example, there is a The Art Institute of Pittsburgh grad who took photographs that were published by The Pittsburgh Post-Gazette. We needed to secure permission from the newspaper, not the grad.
The Photo/Model Release form This form can be used for all interviews and b-roll shots. But if any student artwork is seen in these shots, the Student Permission form or the Artwork Permission form must be signed, depending on the situation.
